This lists all the chart changes the Prescriber has made, plus any notifications of status change for a resident such as discharged.
Facility Filter – if you supply to more than one Care Facility you can use the filter to show all or choose to see changes for only one facility. This lists all chart changes the prescriber has made, plus notifications of status change for a resident e.g., discharged. Follow your pharmacy policy and processes for dispensing chart changes in your pharmacy software system.
Once processed, click on the blue Un-processed button which will change to Processed. The pharmacist checking the changes can click on Checked and this archives the change notification list to the Historic Chart Changes page.
The chart changes can be filtered by clicking on the Un-Processed and Processed for Checking buttons, to view all current changes, click on the All button. To view historic chart changes, click on the button to run the report for Historic Chart Changes within the selected date range.
Go to chart / Print Chart – click on either option as a shortcut to go to the chart or print the chart, use the back arrow to return to the Changes page.
Edit – allows the pharmacy to:
Move the dose time,
Alter the start date for monthly meds to align with packing,
Brand substitute,
Add comments to the charting.
Medications are marked as packed together, packed separately or not packed. Click on No to change the default option.
