This report shows the last reviewed date and history of review for the past 2 years. 

1. Click on Reports from the dashboard.
   
   
    
2. Click on Chart.
   
   
    
3. Click on Historic Chart Review Dates.
   
    
4. Enter the residents name and click on Search.
