MEDICATION REVIEW
To view the list of residents whose charts will need to be reviewed:
Dashboard > Reports > Client Centric Reports > Chart review & reassessment date list.
Open the resident's medication chart and click on Reviewed. This will update the whole chart and records the date and time the review was completed.
If the review is done by a prescriber who isn’t the lead prescriber for this resident, e.g., a locum, then when prompted click No to the question, "Do you want to be assigned as the lead prescriber?"
The Chart Review & Assessment Date List report can provide a summary of the medication chart review dates and the next review date.
MEDICATION REASSESSMENT REVIEW
To indicate and schedule a medical assessment e.g. plan a regular review every 4 to 12 weeks, the prescriber can schedule any medication to have an assessment review date.
The Chart Review & Reassessment Date report provides a list of medication reassessment dates as specified by the prescriber. Prescribers will be notified of any overdue reassessments on their MediMap dashboard.
Click on Reassess in the resident's medication chart.
Choose a date and a comment is required. Click on Save.
The reassessment date will display on the medication line. Reassessments will be listed in date due order in the report with chart review dates renamed Chart Review & Reassessment Date List.
To authorise a single medication line, click Edit on the medication line and then click Save. Your electronic signature will be added to that medication line along with the date and time.
- NIMS
If you have been asked to add NIMs (Nurse initiated Medications/standing orders) for the resident then click on the NIMs tab, tick the appropriate medications for this resident and then click on Add. These medications have been created as a template by a senior staff member at the care organisation.
