Warfarin is charted as a regular medication as it is given each day. It should be charted as an ongoing dose that is updated from time to time via the edit function in MediMap if the INR changes significantly. There is an input field to record the current INR result and next test date if the prescriber chooses to use these.

- Some prescribers will enter instructions in the Indication for charting for nurses advising them of the parameters on when they want to be notified to change the dosing. If within the parameters, set dosing will continue as charted without a change. Other prescribers will review and edit dosing based on lab results. If there is no change to the dose, they will let the facility know.
- Click on Edit to change the dose, record the current INR result and set the next test date.
- Today's dose is always highlighted to let you know when the change will be from today onwards. You can add extra days from the cycling regimen and change the doses for each day in the cycle.
- Once the updates have been saved the facility and pharmacy will be notified.
