Admission of a resident transferring from another facility where a medication chart exists in MediMap.
- Search for the resident by their NHI number/Medicare number. If the resident is associated with another facility a warning will appear, you will need to contact the name facility and ask them to discharge the resident.
- If the transferring resident has already been marked as discharged then the below message will appear, click on Continue.

Only the minimal demographics will display. If this is NOT the resident they are expecting, choose Go Back which will take you back to the search field.
- If this is the correct resident click on Admit to my facility.
- The chart details will display, and an email will be sent to the previous facility notifying them of the residents transfer and admission.
- The admission will be recorded in the resident's facility history and the facility audit log.
- A message will be displayed altering staff to update the resident's details.
If the resident profile does not exist, then click on Add New and follow the usual process for adding a resident to your facility.
Create a residents profile
All staff with manager level access, facility administrator access, RNs or ENs are able to admit a resident to the facility, unless the facility management has blocked this in staff settings.
