Accessing Chart
- Click the Search button and enter the client’s first or last name or NHI.
- Open the chart by clicking on client’s name.
OR
- Click on the Addiction Service button.
- Search for the client and click Go to Chart.
- The client profile details will be at the top of the chart. Scroll down to the OST tab.
- Click on Add New, the funded products available to prescribe within OST services will be displayed.
- If you are unsure of the strength to be dispensed, then select any item and the pharmacy can substitute for another strength if appropriate.
Add other regular medication and PRNs in the appropriate tabs.
- Option 1 - Simple dosing for ongoing maintenance dose
- Start Date – enter a start date or leave as the default Simple dosing
- Daily – Enter the dose
- Dilute – this is optional, tick if appropriate and enter the number of ml. This will display on the chart and on the eScript.
- Option 2 - Titration over one or more dose times
- Start Date – select the start date and the dose type as Titration
- Frequency – select the number of dose times
- Dose – enter the initial dose (mg), choose if titrating Up or Down. Enter the number of mg dose is changing by and the number of days between changes. Enter the end dose (mg), and toggle to Hold or Stop.
- For more than one dose time you will need to repeat this for each dose time.
- Option 3 - Variable dosing, weekly dose cycle
- Start Date – select the date and dose type as Variable Dosing
- Frequency - enter the dose for each day of the week.
- Important Note: You can’t edit an addiction services medicine; you will need to stop and re-chart. This is to stop any confusion with titrated doses and eScript generation. You can edit your comments by clicking on Edit Comment.
- Administration Overview – displays the dispense status for all clients.
- Add/Edit Takeaway – click to change takeaway permissions.
- Pharmacy Info/Days – click to edit pharmacy supply days without re-charting. To add/change a supplying pharmacy click on edit at the top of the chart.

- Click on the SA line to update the Special Authority.
